Conditions Dr. Maks Birikov Treats at Physica Medica

Most people arrive here after trying something else. A clinic that discharged them too soon, a treatment that addressed the symptom but not the source, or a session they spent with an aide rather than their actual provider. At Physica Medica, every session is with me. The assessment, the treatment, the follow-through — one provider, one patient, every visit. That structure is not incidental. It is what makes accurate diagnosis possible, and accurate diagnosis is where lasting results begin.

Spinal

Back Pain, Sciatica & Spinal Conditions

Back pain is rarely just a back problem. Sciatica that radiates down the leg, lower back pain that returns after every round of exercises, stiffness that never fully resolves — these are usually signs that the underlying movement pattern or fascial restriction has not been addressed. I assess the full chain: spinal mechanics, hip mobility, fascial tension, and nerve behavior. Treatment follows the assessment, not a protocol.

01

What I Treat

Lower back pain — acute and chronic, including disc-related and muscular. Sciatica — nerve compression or irritation causing pain, numbness, or tingling down the leg. Herniated and bulging discs. Lumbar and cervical stiffness. Neck pain with or without referred symptoms into the arm or hand. Tension headaches and migraines driven by cervical dysfunction.

Postural

Postural & Structural Conditions

Posture is not an aesthetic problem. Forward head posture loads the cervical spine with forces it was not designed to sustain. A hunched back shifts the rib cage, compresses the diaphragm, and alters how force travels through the entire body. These structural patterns drive pain, recurring injuries, and dysfunction that no amount of stretching will correct on its own. Treatment at Physica Medica addresses the fascial and muscular restrictions that lock those patterns in place.

01

What I Treat

Forward head posture. Hyperkyphosis (hunched back). Scoliosis — managed without surgery, braces, or drugs using Pancafit global postural realignment, the only program of its kind available in the United States. Pelvic imbalances and asymmetrical loading patterns. Postural pain from prolonged sitting or screen work.

Sports & Movement

Sports Injuries & Movement Dysfunction

Recurring injuries are not bad luck. They are a signal that something in the movement pattern is not right — a restriction, a compensation, a load that keeps landing in the wrong place. I treat the injury and the mechanics behind it. That means biomechanical assessment, hands-on treatment for the tissue involved, and strength work programmed around your actual restrictions.

What I Treat

Rotator cuff injuries and shoulder impingement. IT band syndrome. Hamstring, hip flexor, and quad strains. Knee pain including patellofemoral syndrome. Ankle sprains and chronic instability. Trigger points and muscle knots that limit range and cause referred pain. Post-surgical recovery — scar tissue mobilization, strength rebuilding, and return-to-activity planning.

Specialty

Specialty Conditions: Scoliosis, Pregnancy & Vocal Health

Some conditions require more than standard physical therapy training. Scoliosis correction without surgery requires a specific postural methodology — Pancafit, which I am the only provider of in the United States. Pregnancy pain requires adapted positioning and hands-on technique that accounts for the body's changing mechanics trimester by trimester. Voice strain in singers and performers involves the mechanics of breathing, rib cage mobility, and the postural load of performance — not a combination most clinicians are equipped to assess.

  • Scoliosis — postural correction without surgery or bracing
  • Pregnancy pain — back, pelvis, and hip pain through all trimesters
  • Voice strain and breathing dysfunction in singers and performers
  • Diaphragmatic dysfunction affecting spinal stability or breath capacity
  • Fibromyalgia and widespread chronic pain
  • Numbness and tingling from nerve tension or compression

The Model

Every Session Is One-on-One with Dr. Birikov

There are no aides, no technicians, no handoffs. I perform every assessment and every treatment. That is not a selling point — it is a clinical requirement. Conditions that have not responded to previous treatment almost always have a detail that was missed. Catching that detail requires time with the patient, not a template.

Physica Medica operates on a cash-pay model. That means your session time is not dictated by reimbursement rates or insurance visit limits. If you want to understand what that means for your specific situation before booking, call (443) 228-8029.

If your condition is not listed here, or if you have been told your case is complicated, that is a reason to call — not a reason to assume treatment is not possible. I assess what other providers have tried, what has and has not worked, and whether what I do is the right fit for your specific situation. No obligation. Just a direct conversation.
